Hospital at Home for Diabetes and Hypertension Management

Context:

In Kenya, non-communicable diseases (NCDs) are estimated to account for 27% (284,000) of all deaths, with diabetes being responsible for about 10,000 deaths. Moreover, the prevalence of hypertension, another NCD, is 24%, while about 78% of people diagnosed with hypertension are not on treatment.

Hypertension is a significant contributor to cardiovascular diseases, the second most common cause of mortalities in Kenya. Hypertension and diabetes often occur together and share multiple risk factors and causes. Once diagnosed, the patients are confined to managing the illness, which ultimately leads to significant treatment and palliative time in the hospital. It has been observed that patients recover well while at home, and most of the treatments could be undertaken at home using caregivers and scheduled doctor/nurse visits, an aspect known as Hospital at Home.

The concept of Hospital at Home; is increasingly important due to its potential to improve patient outcomes, enhance comfort and convenience, and reduce healthcare system burden. This model offers personalized care, reduces the risk of hospital-acquired infections, and provides a familiar environment with family support.

Additionally, it is cost-effective, optimizes healthcare resources, and enables accessibility and continuity of care through remote monitoring and chronic disease management. By implementing a Hospital at Home model, healthcare systems can significantly enhance the quality of care, improve patient satisfaction, create a more sustainable healthcare system, and ultimately ensure good health and well-being (SDG 3).

Expected output:

Develop a mobile app and web portal for patients and healthcare providers, integrated with wearable devices for real-time data collection. Implement AI algorithms for data analysis and predictive analytics with a dashboard for monitoring the vitals.
